Consultation on draft Carer’s Report forms developed by BAAF Health Group

These forms replace Annexes C and D, and provide a description of the child or young person’s emotional and behavioural wellbeing, in the carer’s own words, and can indicate the need for further assessment.

They have been designed to work alongside the new BAAF health assessment Forms IHA and RHA-C and YP, and should be completed by the carer every time a child or young person has a statutory health assessment, or when they change placement.
